Payment Error Rate Measurement (PERM) - CMS uses contractors to perform statistical calculations, medical records collection, and medical/data
processing review of selected State Medicaid and SCHIP fee-for-service (FFS) and managed care claims.
Recovery Audit Contractor (RAC) - Started as a study in three states, but now is being implemented in all 50 states. Recovery Audit Contractors (RACS)
will be used to detect and correct improper payments in the Medicare FFS program.
Comprehensive Error Rate Testing (CERT) and the Hospital Payment Monitoring Program (HPMP) - CERT measures the error rate for claims submitted
to Carriers, Durable Medical Equipment Regional Carriers (DMERCs), and Fiscal Intermediaries (FIs). The HPMP measures the error rate for the Quality
Improvement Organizations (QIOs).

basic explanations of how to use the Index that I have seen.
American College of Surgeons –PDF file - A manual that is intended to be an explanation of ICD-9-CM and CPT for
Residents. The downloadable manual even contains some cases and shows how they should be coded.
Tulane University Medical Group—Coding and Billing compliance training for Tulane physicians and residences. This
training is the form of PowerPoint Slides and Handouts/Quizzes. The site is geared to physicians and staff at Tulane, but
students may find it useful. There is an entire series on documenting for E/M coding. It also has some training on
Sarbanes-Oxley Anti-Kickback and other health-law related subjects.
